Information related to dental benefits provided by Carpenter Funds may have been impacted by a malicious security breach. This includes first and last name, Social Security number and some combination of other personal or protected health information.

Delta Dental has mailed you a notification letter with steps you can take to avoid potential identity theft.

What you can do: To help safeguard your personal data, Delta Dental has hired Kroll to provide identity monitoring services.

If your Social Security number was potentially involved in the incident, Kroll will provide access to 24 months of identity monitoring services, which includes credit monitoring, $1 Million identity fraud loss reimbursement, fraud consultation and identity theft restoration.
